What You Need to Know If Your Loved One Needs Long-Term Care Support
Caring for a loved one who is facing physical or cognitive decline can be overwhelming — especially when you’re navigating unfamiliar healthcare systems. If your loved one is struggling with everyday tasks like getting dressed, walking, or using the toilet, you may have heard of something called the Severe Disability Assessment.
Here at 1 Physiocare and Wellness, we offer rehabilitation support and work with providers who conduct both centre- and home-based MOH-accredited assessments to help families like yours make informed decisions and receive timely assistance.
What Is the Severe Disability Assessment?
The Severe Disability Assessment is a medical evaluation that determines whether someone qualifies for government financial assistance schemes like:
CareShield Life
Home Caregiving Grant (HCG)
ElderFund
IDAPE (Interim Disability Assistance Programme for the Elderly)
MediSave Care
The goal is to provide monthly payouts or subsidies to help with caregiving, medical supplies, and home care needs for as long as your loved one has a severely disabled status.
Who Needs This Assessment?
If your loved one:
Has difficulty with daily activities,
Is recovering from a stroke, injury, or chronic illness,
Or is becoming increasingly frail with age,
…they might benefit from both the assessment and the support schemes it unlocks.
✅ What Does the Assessment Look At?
The assessment focuses on your parent’s ability to perform six Activities of Daily Living (ADLs):
1. Washing (bathing or showering)
2. Dressing
3. Feeding
4. Toileting
5. Mobility (walking or moving about)
6. Transferring (e.g., moving from bed to chair)
To qualify as severely disabled, your parent must require physical assistance with at least three of the above.
Where and How Is It Done?
Assessments are conducted by MOH-accredited healthcare professionals. At 1 Physiocare and Wellness, we can assist with arranging for:
In-clinic assessments (for mobile seniors)
Home visits (for those who are bedbound or have mobility issues)
The MOH-accredited GP will conduct the assessment and provide further advice on your loved one’s care needs.
